Do you want to make a real difference in people’s daily lives and help shape the future of community-based care? Älvsbyn Municipality is now seeking a dedicated and independent licensed occupational therapist to join our wonderful rehabilitation team within home health care

About the Position

In the role of occupational therapist within our municipal primary care and home health care services, your main mission is to promote independence and quality of life for our patients in their own homes. You will primarily work with older adults as well as people with physical, mental, or cognitive disabilities.

You will base your work on the patient’s own goals and focus on health promotion and rehabilitation. You’ll be part of a supportive team of occupational therapists and physical therapists, but you’ll also collaborate closely with nurses, home care staff, nursing assistants, and family members.

Your main responsibilities:

  • Conduct activity, functional, and environmental assessments in the patient’s home environment.
  • Test, prescribe, and follow up on assistive devices.
  • Plan and initiate rehabilitative and daily-living rehabilitation interventions.
  • Assess the need for and issue certificates for home modifications.
  • Supervise and delegate rehabilitative interventions to nursing staff.

Who are you?

We are looking for someone who is confident in their profession, has a coaching approach, and puts the individual’s needs first. Since the work takes place in the individual’s home, you need to be able to plan and prioritize your workdays independently.

Job Requirements:

  • Licensed occupational therapist.
  • Very good knowledge of Swedish, both spoken and written.
  • A Class B driver’s license is required, as travel is part of the job.

Preferred Qualifications:

  • Experience working in municipal home health care or elder care.
  • Experience with documentation in the Combine medical records system
