The Social Services Department is now seeking a physical therapist with coordinator responsibilities for the Home Health Care rehabilitation unit. In this role, you’ll be an important part of our multidisciplinary coordination team, where you’ll work alongside a nurse, a social services case manager, and a home care coordinator to ensure a safe and seamless transition from inpatient care to the home.
With us, you’ll play a central role, combining your physical therapy expertise with coordination, planning, and close collaboration with patients, family members, and various healthcare providers.

JOB DUTIES
The coordinator team is responsible for creating an efficient and sustainable discharge process that focuses on the individual’s needs for security, continuity, coordination, and safety. The team manages all aspects of Lifecare, from admission notification to discharge.
As a physical therapist with coordinator responsibilities, you will represent the rehabilitation unit on the team and serve as a key link between different departments and professions.
Job Responsibilities
In this role, you will, among other things:
• Represent the rehabilitation unit on the multidisciplinary coordinator team.
• Plan and coordinate discharge processes in Lifecare.
• Collaborate with patients, family members, colleagues, and providers in inpatient and primary care.
• Serve as the point of contact for the home health care rehabilitation unit.
• Process and make decisions regarding home health care as delegated.
• Staff the rehabilitation hotline and coordinator inbox, and distribute incoming cases.
• Contribute to internal collaboration and the development of the unit.
• When necessary and possible, participate in clinical physical therapy work alongside colleagues in home health care.
The position is a daytime role, Monday through Friday.
QUALIFICATIONS
We are looking for someone who
• Is a licensed physical therapist.
• Hold a Class B driver’s license.
• Thrive in a coordinating role with many points of contact.
• Are accustomed to working independently while also valuing close collaboration within a team.
• Demonstrate a professional and respectful demeanor.
• Are organized, flexible, and solution-oriented.
Preferred Qualifications
It is a plus if you have:
• Experience working in home health care or a similar setting.
• Experience with Lifecare.
• Experience in administrative, coordinating, or logistical work.
