
Description of Operations
Home Care consists of seven units: North, South, Near, East, the Support Unit, the Night Unit, and the Resource Unit. All units, except for those in rural areas, are co-located on Upplagsvägen together with home health care nurses, occupational therapists, and physical therapists.
Through interventions tailored to individual needs, we strengthen each person’s ability to manage their daily life and enable them to continue living in a safe and familiar environment.
Job Responsibilities
In your role as a nursing assistant with planning responsibilities, you are responsible for planning approved services for clients living in their own homes. You will oversee and coordinate the team’s daily planning and ensure that the client’s needs and wishes are met in a cost-effective manner. You will support the organization in long-term care planning, ensure follow-up, improve collaborative planning, and be able to handle various tasks simultaneously. Document and report in accordance with applicable procedures.
As a planning-responsible nursing assistant, you will have an outward-facing role, which means you will work closely on a daily basis with family members, nurses, administrators, managers, and colleagues in the operations. You will be involved in driving, following up on, and ensuring the quality of care planning. The position includes spending 50% of your working hours providing nursing and care services according to individual care plans, both as scheduled and during periods of high workload. We prioritize quality and safety for our clients. Client needs and approved services form the basis for care planning. Qualifications
We are looking for someone with training as a nursing assistant or other training that the employer deems equivalent. Experience in home care or similar work is a plus.
You are a patient and communicative person with strong verbal and written communication skills in Swedish. You have good computer skills, are responsible, meticulous, and flexible, and are able to work both independently and as part of a team. You are capable of handling multiple tasks simultaneously and of planning, organizing, and prioritizing in work situations where quick decisions must be made. You are service-oriented and able to interact with both clients and colleagues in a manner appropriate to their respective roles. Great importance is placed on personal suitability.
Prior experience planning services for clients in health and social care is a plus. In our operations, we use, among other systems, Pulsen Combine, TES, Skyresponse, and DF Respons. Under the National Minority Languages Act, Gällivare Municipality is the administrative municipality for Sami, Meänkieli, and Finnish. Knowledge of any of these languages is an asset when applying for employment with Gällivare Municipality.
A Class B driver’s license is required.
Work Hours/Duration
Full-time position (100%). Day/evening/weekend shifts
