The home care activities are located in the department of home care, which consists of six home care units and licensed health care professionals. We are looking for committed and experienced assistant nurses with planning responsibility for the home care activities. Work tasks
In your role as an assistant nurse with planning responsibility in the home care service, you are responsible for planning granted interventions to users in ordinary housing. You ensure and coordinate the daily planning for the home care team and ensure that the user's needs and wishes are met in a cost-effective way. You will support the organization in long-term intervention planning, ensure follow-up, improve co-planning and be able to manage different tasks in parallel. Document and report according to current routines.
As a planning responsible assistant nurse, you will have an outward-looking role, which means that you work daily in close collaboration with relatives, nurses, administrators, managers and employees in the operations. You will be involved in running, following up and quality assuring the intervention planning. The service includes 50% of the working hours performing nursing and care interventions according to individual implementation plans, both planned and in case of workload. We work based on "quality and security" for the users. It is the users' needs and granted interventions that should form the basis for intervention planning. Qualifications
We are looking for you who are a trained assistant nurse or have other documented experience that we consider equivalent. You also have experience of home care or similar activities. You are an educational and communicative person with good ability to express yourself in Swedish in both speech and writing. You have good computer skills, are responsible, careful and flexible and have good ability to work both independently and in teams. You have the ability to manage different tasks in parallel and to plan, organize and prioritize in work situations where quick decisions must be made. You are service-oriented with the ability to meet both users and employees based on their different roles. It is an advantage if you have previously worked with planning interventions for users in health and social care, have knowledge of work in intervention planning tools eg. TES intervention planning, TimeCare planning. Great emphasis is placed on personal suitability.
Driving license B is a requirement.
In the framework of the National Minority Languages Act, Gällivare municipality is the administrative municipality for Sami, Meänkieli and Finnish. Knowledge of one of these languages is an advantage when applying for employment in Gällivare municipality.
