Description of the workplace:

Adaptive services in Gjøvik municipality consist of 12 departments that encompass a diversity of services for users with various functional disabilities. The service area has broad professional expertise with an interdisciplinary profile both in management and in the employee group.

Description of workplace /Åslundmarka 1:

The department of adapted services department 1 in Åslundmarka is responsible for municipal health and care services for people with disabilities. This includes practical assistance/training, health services and coordination of various services. The department is divided into groups in terms of service provision

There is a vacancy for a permanent position in the group of residential communities and ambulatory environmental worker service. The user group is mainly people with mental illness, some have developmental disabilities. The services are provided as full-time services in residential communities at Åslundmarka care center or assigned hourly scope in outpatient activities in Hunndalen and Nordbyen. Services for individuals are provided in collaboration with other support services within the 1 and 2 line services.

Work tasks:

- Provide health and care services in users' homes.
- Assist and provide training in activities of daily living.
- Medical follow-up, according to delegated competence.
- Interdisciplinary collaboration with colleagues and other support services within the 1st and 2nd line services
- Participate in meetings internally and with external partners.
- Primary contact function and coordination of services
- Preparation of plans for individual users, individual goals and measures: 35.5 h/week with day and evening shifts. Work every 3 weekends with ordinary shifts or every 4 weekends with long shifts, as desired.

Qualifications:

- Authorization as a health professional.
- Relevant further education is desirable, but not a requirement.
- Relevant work experience from services to the user group.
- Knowledge of the principles of targeted environmental work.
- Knowledge of user participation, the Health and Care Services Act and the Patients' Rights Act is an advantage.
- Competence in the use of computer tools.
- Knowledge of patient record systems is an advantage, but training is provided.
- Good collaboration skills and ability to work independently
- Personal suitability and awareness of the role as a professional, colleague and employee.
- Ability to innovate and flexibility.
- Salary according to agreement and based on competence.
- Driving license for car.
- Approved police certificate.
