Working hours are 37.5 hours per week, daytime shifts, currently daytime with weekends and holidays off.

Tasks:

The work is varied and combines relationship work, follow-up and coordination
Tasks will include:

  • outreach and outpatient follow-up of patients
  • empowerment and motivational conversations
  • practical assistance and health care
  • mapping, goal work and action follow-up in collaboration with the patient and network
  • coordination of services and collaboration with other agencies
  • support for activity, treatment, work and social participation
  • family and network work
  • documentation and record keeping in DIPS
  • participation in professional and development work in the team
Qualifications:
  • Nurse or social worker, other health or social bachelor's degree can be considered
  • relevant further education in mental health work and / or substance abuse disorders is desirable
  • work-related experience from services to the user group
  • knowledge of the Health and Care Services Act, including Chapter 10 of the Act
  • competence in the FACT model is an advantage, but training will be provided
  • requirement for an approved police certificate
  • driving license class B
