Working hours are 37.5 hours per week, daytime shifts, currently daytime with weekends and holidays off.
Tasks:
The work is varied and combines relationship work, follow-up and coordination
Tasks will include:
- outreach and outpatient follow-up of patients
- empowerment and motivational conversations
- practical assistance and health care
- mapping, goal work and action follow-up in collaboration with the patient and network
- coordination of services and collaboration with other agencies
- support for activity, treatment, work and social participation
- family and network work
- documentation and record keeping in DIPS
- participation in professional and development work in the team
Qualifications:
- Nurse or social worker, other health or social bachelor's degree can be considered
- relevant further education in mental health work and / or substance abuse disorders is desirable
- work-related experience from services to the user group
- knowledge of the Health and Care Services Act, including Chapter 10 of the Act
- competence in the FACT model is an advantage, but training will be provided
- requirement for an approved police certificate
- driving license class B
