
Tilrettelagte tjänster is part of the service area health and care, and provides home-based services to adults/younger people with various disabilities. The user groups are people with mental illness and/or substance abuse problems and people with various disabilities. Health care and practical assistance/training are provided to those who need it, ranging from one-hour follow-up in the home to full-time care in shared housing.
Mental health work is part of the adapted services service area. In the service area, health care, practical assistance and coping sessions are provided to users with various disabilities, including people with mental illness and/or substance abuse problems. This position is currently affiliated with a 24-hour residential community for people with mental illness. Services to the individual user are provided according to individual decisions based on the Health and Care Services Act.
Tasks:The service mental health work and environmental work is provided both in co-located housing for people with mental illness and in ambulatory follow-up services in the home. The positions are currently associated with a 24-hour residential community for people with mental health challenges.
The service is under development, and an interdisciplinary approach and innovative thinking will be emphasized
Important focus areas are assistance to the individual user in the form of coping conversations, motivational work and coordination of various services. In addition to practical assistance and training, with different needs for assistance in daily life skills.
Collaboration with other follow-up services
Primary contact function and coordination via responsibility groups/ IP coordinator
Drawing up plans for the individual user, including individual goals, measures and individual plans.
Socialization and networking
Rotation (day/evening) with work every 3rd weekend
Qualifications:
Authorization as a nurse or social worker. It is desirable with further education in mental health work and/or substance abuse. Applicants with other relevant further education, or prior learning, may be considered.
Good experience with and commitment to the various user groups is required
Good documentation skills, as well as good oral and written communication skills are required
Good collaboration skills, in addition to being able to work independently
Ability for targeted and systematic follow-up work
Personal suitability is given great weight
Ability for restructuring, innovation, and flexibility is emphasized
Driver's license on car
Requirement for approved police certificate
