Description of the workplace:

The position is part of the health promotion services and affiliated with the Home Rehabilitation Response Team (IST), based in Øverby. Health promotion services help residents remain active and independent in their own homes for as long as possible.

IST is a mobile, interdisciplinary team consisting of a nurse, a physical therapist, and an occupational therapist. The team offers targeted, time-limited rehabilitation in the home for people with somatic health challenges and the potential for functional improvement. The work focuses on strengthening functional ability, independence, and participation through user involvement, personal effort, and self-mastery.

IST contributes to:

• Early intervention and appropriate rehabilitation at the right time.
• Safe transitions from specialized health care or municipal short-term stays back to the home.
• Effective collaboration with services and departments that provide follow-up care for the user within the municipality.

Job Responsibilities:

• Assume professional nursing responsibility for the assessment, evaluation, and follow-up of users in the Intervention Team, based on the ICF, ADL, and overall health status.
• Prevent, identify, and follow up on health challenges that may affect function, activity, and participation.
• Work in a knowledge-based manner, taking into account the user’s goals, resources, and coping abilities.
• Guide and support clients and their families to promote self-management, motivation, and active participation in the rehabilitation process.
• Initiate, coordinate, and participate in interdisciplinary collaboration to ensure comprehensive and coherent services.
• Contribute to professional development, quality improvement, and the development of rehabilitation services.

Qualifications:

• Continuing education totaling at least 60 credits in rehabilitation or another relevant field for specialty nurses.
• Experience in the field of rehabilitation and municipal health and care services is desirable.
• A solid understanding of the nurse’s role in the rehabilitation process, including goal setting, implementation, evaluation, and completion of interventions.
• Interest in and expertise in professional development, quality improvement, and service development.
• Good Norwegian language skills; minimum level B2 for applicants without a Scandinavian language background.
• Valid nursing license.
• Class B driver’s license.
• A valid police clearance certificate must be submitted prior to starting the position.
