Health and Coping Services in Hamar Municipality are evolving in response to new responsibilities and to meet changes in the population’s needs, medical advancements, and new tools. The Health and Coping Service aims to facilitate greater coping for users and provide high-quality services to those who need them. It is important for both the individual and society to utilize the resources, abilities, and opportunities that residents themselves possess to manage their daily lives.
The Unit for Rehabilitation, Intake, and Palliative Care provides services to adults and older adults living at home. The field represented by the unit is a key contributor to the effort to shift the professional focus of services toward early intervention and self-management.
The unit is multidisciplinary, comprising nurses, skilled care workers, physical therapists, and occupational therapists, and is tasked with providing interdisciplinary assessment and follow-up/rehabilitation for adults and older adults who have, or are at risk of developing, limitations in their physical, mental, or social functioning. Part of the team assists patients in the final stages of life. The team is divided into three groups: an intake/intervention team, a rehabilitation team, and a palliative care team. The team is currently based at Prestrud Center and Finsal Center. This position is in the palliative care team.
