Right now, active development work is underway with a focus on quality and improved social documentation, where you as an employee are expected to contribute and be involved. - Provide care and support to the elderly based on implementation plans during short periods of care. - Receive, understand and translate information when admitted to and discharged from hospital and other care providers. - Document interventions, observations and deviations according to current legislation and procedures. - Observe changes in health status and report to the nurse and colleagues. - Participate in rehabilitative work methods and collaborate with assistance administrators, nurses, physiotherapists/physiotherapists and occupational therapists. Participate in follow-ups of interventions and goals. QUALIFICATIONS We are looking for you who: - Is a trained assistant nurse and has the ability to work independently and follow implementation plans and care-related efforts. - Have very good knowledge of Swedish, both in speech and writing. - Can document clearly and correctly in Swedish and have basic digital skills. - Has good observation skills, is responsible and can report orally in a clear and structured way. - Has good cooperation and communication skills, likes to work in teams and follows established routines. Interacts well across professions, participates actively in rehabilitation efforts and contributes to follow-up and documentation.
