We believe that individual and closer follow-up of individual patients can contribute to more people being able to live at home longer and be discharged earlier from institutions. We are looking for a solution-oriented person who works flexibly to ensure good patient transitions and processes. The position will have a work schedule with weekend work.
Tasks
- Environmental therapeutic work, rehabilitation and habilitation in line with current requirements for quality, legislation, municipal guidelines and professional good practice
- Maintain the patient's resources and provide individual follow-up tailored to the individual's needs, resources and must
- Medication management
- Map, implement and evaluate measures in line with everyday coping and user participation
- Supervise, teach and collaborate with patients, relatives, colleagues and students
- Contribute with innovation and in development and improvement work
- Participate and contribute in interdisciplinary collaboration
- Document in professional systems
- Keep up to date in their own field

Qualifications
- You must have Norwegian authorization as a social worker. Other relevant three-year health education may be considered. Please state your HPR number in your application
- You must have experience in mapping and assessing the need for help and the level of care
- Knowledge of rehabilitation, everyday coping and focus on user participation is required
- You must have generally good ICT skills and a willingness to learn relevant professional systems
- Good oral and written presentation skills in Norwegian
- You must be able to present a valid police certificate on appointment, cf. the Health and Social Care Services Act. Helse- og omsorgstjenesteloven § 5-4 before starting
- Driver's license
