We are seeking two social care workers for permanent, full-time (100%) positions in the Outpatient Team 24+ in Nordre Land Municipality.
These positions involve weekly daytime work. However, there may be a need to work evenings, weekends, and possibly nights at times—particularly during the initial launch of services and the assessment phase.
The positions will involve close, interdisciplinary collaboration with many different services and departments. We work with people with disabilities of a physical, mental, somatic, and social nature. Services are primarily provided in the user’s home, regardless of where they live; in some cases, support is also provided outside the home.
Job Responsibilities
- Follow-up and evaluation of decisions.
- General environmental therapy work.
- Serving as an active driving force within the profession and the community, with responsibility for providing guidance.
- Responsibility for coordination and interdisciplinary collaboration.
- Development of plans, systems, and tools.
- Medication management.
- Accompanying service recipients to doctors or other healthcare providers.
- Using assessment tools as a systematic and structured method to identify changes.
- Collaborating with family members, colleagues, and external partners.
Qualification Requirements
- License as a social care worker.
- Preferably further education in mental health, habilitation and rehabilitation, or social work.
- Experience with goal-oriented social work and behavioral analysis.
- Experience developing plans and using relevant systems and tools in daily work.
- Good knowledge of relevant legislation.
- Knowledge of counseling.
- Experience building relationships with clients.
- Experience collaborating with relatives and family members.
- Class B driver’s license.
