Place of work:
The position is part of the municipal area health and coping. The municipal area's goals and strategies focus on coping, independence, belonging and participation. The allocation unit is located at Labo in Lena. The office is responsible for two distinct areas, service allocation and housing management. In addition, the office functions as a coordinating unit.

Team 1
Is responsible for the allocation and administration of services within mental health and substance abuse, children and young people, as well as cases involving people with various disabilities.

Team 2
Is responsible for the allocation and administration of somatic health and care services.

Housing office
Housing management involves the management and allocation of municipal housing, as well as the Housing Bank's instruments, and social housing follow-up.

Coordinators
Children's and system coordinator for IP and coordination, as well as cancer and dementia coordinator.

Coordinating unit
The coordinating unit will help to ensure comprehensive and coordinated service provision. The unit has overall responsibility for the work with individual plans and coordinators, as well as training and follow-up of the coordinators in the various services.

There is a vacancy for 1 x 100 positions as a coordinator for children, young people and adults in need of long-term and coordinated services.

The working hours are 37.5 hours per week with daytime work.

About the position:

The position of coordinator will take care of the coordinator function for individuals who are entitled to a coordinator/individual plan (IP) as a result of the need for long-term and coordinated services under the Health and Care Services Act.The coordinator must ensure the necessary follow-up of the individual user or patient, as well as ensure coordination of the service provision and progress in the work on the IP.To coordinate means "to make work together", which will involve seeing services in context in order to achieve wholeness and coherence in the offer.

Better interdisciplinary efforts (BTI) is a focus area in Østre Toten municipality. The aim is to ensure that children, young people and their guardians receive a holistic and coordinated effort with a focus on help that helps.

We are looking for you who have experience of working with people in need of coordinated services, and who have a desire to make a difference for children and young people and their families, as well as adults with complex needs. Guidance, follow-up and coordination of services are among the core tasks, and you will have the opportunity to actively contribute to the work of further developing the function.

The position is a temporary position in the period 01.08.2026 - 31.03.2027.

Tasks:

  • Coordination of the overall provision of services for individuals.
  • Have the function of coordinator for children, young people and adults in individual cases who have a decision on a coordinator/individual plan under the Health and Care Services Act.
  • Be proactive for progress in the work with IP.
  • Maintain regular contact with the service recipient and the service system in order to have an overview of the planning process and ensure follow-up, including assisting the service recipient in meetings with the support system.
  • Ensure cooperation with the service recipient and relevant partners on the distribution of responsibilities, progress and evaluation of measures.
  • Chair meetings in responsibility groups.
  • Help to ensure that children, young people and adults participate in their own development and follow-up/treatment.

Changes in work tasks must be expected, as well as having to work across the teams.

Qualifications:

Requirements:

  • Formal education: 3-year relevant education from college or university.
  • Minimum 2 years of experience from the municipal sector or specialist health service.
  • Good command of Norwegian orally and in writing, Bergen test minimum B level.
  • Driver's license class B.
  • Good digital skills.
  • Approved police certificate.

Desirable requirements:

  • Experience working with IP and coordination.
  • Experience in case management and coordination.

  • Relevant further education is desirable.
