The health and care services in Østre Toten are facing major and exciting development processes.

Through "New Welfare 2040", the home care service is focusing on public health and preventive efforts. We want to help create a community municipality, where residents experience an inclusive local community and are given the opportunity to take care of their own health and master their own lives. Collaboration with relatives and volunteers is key, and we are constantly adopting new welfare technology aids.

The home care service in Østre Toten municipality is located in Labo care center. As an employee of the home care service, you have access to a company car with associated parking garage and there is free employee parking. The home care service is organized in 3 departments, and has a joint night service with the institutional service. Each department in the home care service has its own business manager. In addition, there are specialist managers in the departments who facilitate good patient flow in everyday work. There is cooperation between the short-term department, the physiotherapy and occupational therapy service and the allocation unit to ensure good patient pathways.

The home care service has many different patients who give our employees new and exciting tasks. The user group is people of all ages with different challenges.

The schedule is day and evening shifts and work every 3rd weekend. You will have the opportunity to influence your own everyday life.

Tasks:

• Home care based on the individual's needs.
• Follow-up of security alarms and emergency medical assessments of patients with changes in the disease picture.
• First-time visits, assessment and mapping of patients' coping levels.
• Documentation and follow-up of patient records.
• Collaboration with GPs, relatives, the short-term unit and other municipal agencies.
• Primary/secondary responsibility for individual users.
• Defined areas of responsibility in the department/unit based on needs, expertise and areas of interest.
• Supervision of colleagues, apprentices, students and new employees.

Qualifications:

Requirements:

• Authorization as a health care worker/assistant nurse.
• Driver's license class B.
• Approved police certificate.
• Good knowledge of Norwegian orally and in writing.

Desirable requirements:

• Experience from municipal health and care services.
• Experience from home-based care.
• Good digital skills.
