Kiruna Health Center is one of Norrbotten's largest health centers with approximately 16,500 listed patients. We are around 60 employees who work as a team to meet our patients' needs for first-line healthcare.
We are looking for
We are looking for you who hold a license in a healthcare profession, for example you who are a nurse, occupational therapist or physiotherapist. We also see that you have good knowledge of both Swedish and English, in speech and writing. We see it as meritorious if you have additional language skills in Finnish or Sami and/or if you already have work experience in LifeCare.
We put great emphasis on your personal characteristics. You are independent, creative and communicative as you listen and are receptive to the other party and adapt to the situation. You also have a good overall view and cooperation skills as you value and see the benefit of working together with others.
You get to work with this
As a coordinator, you are the spider in the web for patients who need help at home upon discharge from inpatient care and in outpatient care. You are responsible for carrying out a self-planning with the patient's needs in focus. When there is a need for help, you are in charge of coordination for calling and holding coordinated individual planning meetings (SIP) together with the municipality's various units and other outpatient care units. You work in the communication tool Lifecare, where you are also user support for other colleagues within the system. As a coordinator, you work with the patient in focus in close collaboration with staff in primary care, psychiatry and the municipality.

Information about the service
The position is a permanent, full-time position. Working hours are shifted to daytime. Access is by agreement. Selection and interviews take place continuously and the position can be filled before the application deadline.
Region Norrbotten applies individual wage setting according to collective agreements.
