Är du den rätta? The dementia team in Umeå works in both ordinary and special housing to promote safe care and independence at home.
The team does not provide its own interventions, but supports, for example, relatives, home care and caregivers through mapping, advice, guidance and treatment plans.
In ordinary housing, we support, for example, the relative or home care service in finding strategies in everyday life to enable the person to remain at home.
In care homes, it is common for the person to have behavioral and psychological symptoms (BPSD) and then we focus on mapping them, creating response plans and planning for measures that reduce the symptoms and increase the person's well-being. Together with family consultants, we make home visits to map out how both the person with dementia and the family member will be supported. The guiding principle is the vision "Together for a better life for life" with a focus on collaboration, prevention, competence and digitalization. The team consists of dementia nurses, assistant nurses, occupational therapist and physiotherapist, with office workplace at Dragonen.

Your work tasks
The main tasks include home visits, observation, consultation, guidance, advice and training. The role involves always having the person with dementia and their relatives in focus, as well as supporting and instructing staff and relatives in their daily work.
You will be expected to listen to and promote your own and others' ideas, and actively share knowledge within the team and with others in the services. The role involves responsibility for own professional development in dialog with the manager and commitment to continuous learning and development in the field.
The assignment includes visiting elderly care homes and care recipients in home care services in Umeå's districts and surrounding villages. The dementia team also supports home care staff at the private providers and care staff in disability care in LSS homes, housing support and personal assistance for dementia. Travel in the service is done by leasing car, bus or electric bike.
About you
We are looking for you who are trained as a dementia-specialized assistant nurse, either through a vocational college or as a Silvia sister through Silviahemmets högskola. You have a few years of work experience after completing specialized training in activities for people with dementia in the region or municipality. Furthermore, you have experience of working with Word, Powerpoint and Excel.
Meritorious for this position is if you have:
Training in RESIMA (Resources in the Meeting with Aggression) or equivalent, BPSD, documentation/language ombudsman.
Experience of working with and guiding relatives.
Experience of planning and implementing training initiatives independently and together with a colleague.
Interest in planning and implementing improvement work aimed at ensuring that people with dementia receive safe and secure care and their relatives receive support and guidance on strategies in everyday life.
Knowledge and understanding of how dementia affects family members.
The position requires initiative and independent work, while cooperation with colleagues, relatives and other professionals is essential. Flexibility is important as situations can often change quickly. You are communicative and adaptable, which allows you to guide and train groups both on site as well as digitally.
The position requires you to have an excellent command of spoken and written Swedish and a good knowledge of English, you also need to have a B driving license.
Furry animals may occur in the workplace so you should not be allergic.
