Your role
As a Home Health Care Nurse, you will provide health care services at the primary care level for patients aged 18 and over. In your role as a nurse, you will need to take initiative and prioritize between different interventions. You work closely with patients, relatives, nursing colleagues, occupational therapists, physiotherapists and other key stakeholders, both within the municipality and the region, to create the best possible conditions for safe and quality care in patients' own homes.
Your new workplace
Health and medical care in the home is responsible around the clock for home care, rehabilitation, habilitation and assistive devices in ordinary housing for people over 18 who need health care interventions.
We work based on a person-centered approach and a holistic approach where each patient meeting is unique and significant. Our goal is to strengthen the individual's ability to live an independent life and offer good and safe care with high quality. The work is based on a health-promoting and rehabilitative approach with a focus on participation, coordination and clear division of responsibilities.
We care about sustainable working hours and work actively to create healthy schedules adapted to the needs of the business. As a nurse in home health care, you work mainly during the day with sparse evening and weekend duty (every 6th weekend).

About you
We are looking for you who are a registered nurse. Experience and / or further training in districts, care of the elderly or psychiatry is an advantage. You have good experience of working in digital systems and express yourself well in Swedish, both in speech and writing. A B driving license is required for the position.
You thrive on independent work while valuing collaboration and having the ability to coordinate and keep together care efforts. You work with a professional and positive attitude towards both patients and colleagues. Great emphasis will be placed on personal suitability.
In the workplace there may be furry animals and people who smoke. Heavy lifting occurs, transfer aids must be used.
