Umeå municipality is looking for a strategic and collaborative project manager who wants to be the hub in the implementation of the co-morbidity reform. The reform aims to create a coherent, needs-oriented and person-centered care and support structure for people with co-morbidity.

About the business

The support and care administration is responsible for three central areas of activity: authority exercise, disability care and individual and family care. Our mission is to create the conditions for people to live a safe and independent life.

Within the administration there are three perspectives: finance, HR and quality. Within the quality perspective, there is the Unit for systematic quality work, which is tasked with participating in and creating the conditions for systematic quality work, in accordance with the Board's management system for systematic quality work

The unit consists of employees with various assignments within the systematic quality work - coordinators, socially responsible coordinator (SAS), medically responsible nurse (MAS), process manager, coordinator for the transition to the new Social Services Act and children's rights strategist.

Now a new role is being created - a project manager for the implementation of the co-morbidity reform, which will also belong to the unit.

Your duties

As a project manager, you will have overall responsibility for planning, driving and following up the municipality's development work linked to the co-morbidity reform. The reform aims to strengthen coordination between social services and health care and ensure that people with complex needs receive coherent, accessible and long-term sustainable efforts.

You will:

  • Lead the work of establishing and developing common structures, working methods and processes between the municipality and the region.
  • Coordinate efforts together with the surrounding municipalities to create equivalence and efficiency throughout the area.
  • Ensure that the intentions of the reform are implemented in the operations, from strategic level to practical implementation.
  • Drive analysis, improvement and change work to create a more integrated and person-centered care and care.
  • Follow up results, quality and effects and report to the steering group and management.
  • Contribute to creating a common goal image and culture that promotes collaboration between the principals.
  • Watch the world and disseminate knowledge about legislative changes, guidelines and national assignments linked to the reform.

About you

We are looking for you who have:

  • Academic education in social work or other relevant focus.
  • Experience in project management or strategic development work.
  • Documented experience of collaboration between municipality and region or in areas related to mental illness, addiction, addiction or social services.

It is meritorious if you:

  • worked with the implementation of national reforms
  • have experience from municipal social services.

  • Have very good communication skills and a confidence in presenting and anchoring decisions.
