Elderly care in Umeå municipality wants to be a resource that frees and strengthens people's ability to live an independent life in proximity and interaction.
We are now looking for three coordinators (social workers) who want to work this summer!
As a coordinator with a focus on housing placement, you will be a central link in the process of matching the individual's needs with the right place in special housing. You will collaborate with other team members to develop and implement digital solutions that support the entire chain from application and investigation to offer and move into special housing. You contribute to creating clear and legally secure processes for housing placement, where digital tools are used to streamline the flow of information, follow up waiting lists and facilitate communication between administrators, housing placement officers and operations.

The Digital Care unit is currently responsible for the installation of digital solutions in the home and for supporting people in the use of digital technology. We already have IT technicians who work practically with installation, service and support.

Duties and responsibilities
As a Housing Placement Coordinator, you will be responsible for working closely with case managers and other stakeholders to ensure that people with special housing decisions receive the right support throughout the process. You will keep up to date with waiting lists, offer housing based on the individual's needs and have initial conversations with the individual and relatives to inform about the process and create security before moving.
The work also involves gathering additional information, documenting decisions and following up on cases where someone declines an offered place. You contribute to developing and improving routines and digital tools for housing placement, always with a focus on legal certainty and individual adaptation according to current legislation.
In addition to this, other tasks may also be relevant such as collaborating in the digitization team, supervising colleagues, participating in development work and contributing to creating innovative solutions for the future of elderly care.
Qualifications
Degree in social work or equivalent higher education.
Good knowledge of the Social Services Act, publicity and confidentiality and administrative law.
Experience of handling and professionally dealing with people in vulnerable situations.
Very good digital skills, including the ability to manage relevant systems, digital tools and platforms independently and effectively.
Very good written communication skills in Swedish, including the ability to write clear, structured and appropriate reports, supporting documents and documentation.
Ability to lead, collaborate and drive change in an organization.
Strong interest in digital development and innovation in health and social care.
Flexibility and ability to work both independently and in teams.
B driving license.

Working hours may vary according to business needs and may include working outside office hours.
